Summary: The Arc of Union County Inc.
is a leading provider of Person-Centered services to individuals with intellectual and developmental disabilities through Adult Day Services programming.
Day Habilitation programming provides education and training to acquire the skills and experience needed to participate in the community, consistent with the participant’s Service Plan.
This includes activities supporting participants with building problem-solving skills, self-help, social skills, adaptive skills, daily living skills, and leisure skills.
Activities and environments are designed to foster the acquisition of skills, building positive social behavior and interpersonal competence, greater independence, and personal choice.
Support Professional Sub, Adult Day Services engages service recipients in small or large group outings to the community, and leisure activities.
Support Professional Sub, Adult Day Services provides a safe, secure, and dependable support and assistance in areas that are necessary for service recipients to achieve full social inclusion, independence, personal and economic well-being as approved by the Individual Service Plan (ISP).
The Support Professional Sub, Adult Day Services focuses on improving the quality of life of service recipients.
The incumbent implements and facilitates behavioral/habilitation plans, teaches and monitors skills development, provides support and mentoring for all service recipients through a person centered approach.
The incumbent considers the unique characteristics and needs of the individual as expressed by the individual and others who know the person, such as family, friends, service providers, etc.
The incumbent ensures services are provided in a non-intrusive manner, and service recipient is included in problem-solving and decision-making.
The incumbent is to maintain a positive, healthy, and stimulating environment for service recipients and assures adequate supervision at all times.
The incumbent must follow assigned schedule and agency clock in/out procedures.
The incumbent must be available to work Monday through Friday, between 8:30am to 3:00pm as needed, or with flexibility of hours such as 8:30am to 2:30pm; 9:00am to 2:30pm; 9:00am to 3:00pm etc.
This is a part time position.
The incumbent reports to the Assistant Director, Adult Day Services or designee.
This position requires completion of DDD System Mandatory Training Bundle within 90 days of hire, DDD and Agency Orientations, other designated trainings and a minimum of 12 Hours of Professional Development completed annually.
This position requires that information related to service recipient, employee, program operations is handled, channeled, managed and delivered professionally to the appropriate and right audience or party consistent with agency and department policy.
Skills: This position requires Ability to use Windows software, Ability to use a data entry software such as Therap, Ability to complete Shift and Individual Served ISP Data entry in web based electronic platform.
Willing and able to learn all areas of support, service and function outlined herein; having some familiarity, knowledge or experience in Social Supports Services/Individuals with developmental disabilities, or Adult Day Habilitation Supports, Home Health Aide/Nursing Assistant /Private Personal Care experience.
Must be able to successfully complete all required training programs and be able to follow directions and complete all assigned responsibilities.
Must be able to read, write and speak English.
Basic computer and math skills required.
Qualifications: Minimum high school diploma or equivalent.
Must be 18 years of age or older.
One year Day Habilitation or Residential in program Direct Support Professional experience working with individuals with developmental disabilities.
Required a Valid New Jersey driver’s license and abstract (not to exceed 5 points), and ability to meet agency insurability requirements.
Complete State/Federal Criminal Background checks and Central Registry checks.
Major Position Responsibilities Be flexible and available to meet the arising staffing needs of Adult Day Habilitation by filling in shifts.
Must be able to work a minimum number of shifts/hours as required monthly by the agency.
Fulfill obligations to pre-assigned shift(s) and adhere to assigned schedule.
Contact Assistant Director, Day Habilitation, periodically, to communicate availability and/or inquire about availability of shifts in need of substitute coverage.
Maintain open line of communication by responding to calls, messages; communicate and provide changes related to phone numbers, email, and home address as needed.
Implement the strategies necessary to assist the individual in progressing toward his/her personally defined outcomes as indicated in the ISP.
Record data (in Therap) relative to assigned outcomes for service recipients, including ISP tracking, Daily Training Record (DTR) etc.
Document and communicate to Supervisor, Adult Day Services any changes on the service recipient needs or functional abilities.
Support and assist service recipients in educational, enrichment or recreational activities as outlined in his/her Service Plan to enhance inclusion in the community.
Provide assistance with Activities of Daily Living, Increasing Community Participation, Increasing Independence, Safety Awareness and Learning Activities.
Implement skill development, Behavior plans, IHP and other plans as developed and directed.
Log into Therap Daily and review all logs, Email, & S.
Comm as required, rotate daily in attending to or supporting different individuals in program, rotate in various shift tasks namely: Rotation in community activities facilitation, Rotation in Med Administration & Meal prep, Rotation in Hygiene Support & Life Skills Training, Rotation in program cleaning, Rotation in Therap Data entry, Rotation in checking off on shift tasks as designated in Therap etc.
To ensure that the agency is able to bill Medicaid for the services provided, during shift or before the end of shift enter/input ISP in Therap Daily for the service (s) provided to the service recipient directly during shift i.

Correct any missed entries.
Follow the developed weekly shift assignment schedule outlining daily staff rotations in assignments.

Facilitate Life Skills sessions and meetings such as Self Advocacy as planned for service recipients.
Assist the individuals served with daily living skills including development of self-control, independence, enhancement of social, communication, interpersonal, adaptive and functional skills (e.
g.
feeding, toileting) in accordance with their individualized plans and needs.
Complete all required documentation and administrative assignments as directed and in accordance with Agency standards and the Division of Developmental Disabilities.
Ensure service recipients fulfill all scheduled events such as computer training, exercise, art, music, therapy, and community integration/education trips.
Provide assistance to individuals during community trips, and assure the safety and supervision of participants.
This encompasses providing appropriate level of support to service recipient during events, remaining with service recipient throughout the event thus providing direction, guidance and care as required.
Professionally and effectively interact with families, supervisors, co-workers and others.
Complete all required documentation relative to vehicle or transportation services.
Ensure that Agency vehicles are driven in a safe manner.
Ensure proper administration of medications and completion of all appropriate documentation; monitor individuals served after medication administration.
Comply with medical, dietary and other physician orders as stated; report any arising medical concerns in accordance with policies and procedures.
Responsible for writing the date, the time and their initials on the bubble pack/bingo cards after administering medication.
Maintain regular, reliable, and non-disruptive attendance is an essential job duty.
