Now Hiring - Bereavement Coordinator (Part Time/ PRN)
The Bereavement Coordinator, will be responsible for but not limited to:
Provides bereavement services to individuals and families of hospice patients, in accordance with Ascend Hospice policies and procedures and applicable laws and regulations
· Performs comprehensive initial bereavement assessments
· Assesses need for counseling related to risk assessment for grief
· Assesses bereaved’s psychosocial status, potential for risk of suicide
· Provide counseling for the purpose of helping to resolve symptoms related to grief and loss, psychosocial stress, or difficulties coping with social environment
· Provide counseling for purpose of enhancing, protecting, or restoring the capacity for coping with the social environment
· Conducts ongoing reassessment of bereaved’s needs
· Provides emotional support, short-term crisis intervention and individual and/or family counseling when indicated
· Assesses special needs related to cultural diversity including communication, space, role of family members and special traditions
· Possesses knowledge of stages of development through a lifetime and uses this knowledge when assessing and counseling bereaved
· Refers family and friend to assigned chaplain for spiritual assistance
· Maintains contact with appropriate community agencies and services in order to promote interagency cooperation and to facilitate related referrals
· Collaborate with other disciplines
· Facilitate caregiver support groups and/or bereavement support groups – virtual or in-person
· Conducts home visits to bereaved who require in-person bereavement counseling
· Attends Interdisciplinary Team Meetings to collaborate with the team regarding patient and caregivers Pre-Bereavement and Post-Bereavement needs
Efficiently performs administrative support responsibilities related to bereavement services, in accordance with Ascend Hospice policies and procedures.
· Complete electronic documentation in the electronic medical records system according to Ascend Hospice Protocol
· Ensures compliance with care plan contacts for bereaved according grief risk and contact frequency
· Prepares mailing labels and letters for bereavement mailings to bereaved family and/or friends in the scheduled time frame (30 days, 3 months, 6 months, 9 months, 13 months)
· Follows verbal and written instructions accurately and in a timely manner
· Creates bereavement reports from HPMS system
· Audits to maintain compliance
· Completes documentation of bereavement calls according to compliance guidelines
· Recognizes and performs duties which need to be performed although not directly assigned.

Education:
High School Diploma or Equivalent
Professional Experience:
2-4 years of related experience
Strong Organizational Skills
Clerical/Administrative experience
